  • Plan runs 12 months from the effective date of membership.
  • Payment is due in full at time of service for any treatment not covered 100%.
  • This plan is only accepted at this dental office. Therefore, if you are referred to a specialist,

  • This plan is non-transferrable to other family members.
  • Dental Products are not included in the plan.
  • This plan is non-refundable, and services cannot be rolled into the next membership renewal if not used.
